Don’t have your Health Professional's health code?

You must contact their clinic to obtain their health code. The health code identifies your practitioner and ensures your health assessment results go directly to them, and only them.

Using an incorrect access code will either prevent you from accessing the online assessment or connect you with the wrong practitioner and necessitate you re-doing your assessment.

Don’t have a Health Coach^® Health Professional?

If you have not been referred to our site by a health professional, please email us at hpsupport@healthcoach.com or call us at 1 888 888 8565 and we will assign you to one of our Health Coach^® Nutritional Consultants and provide you with their health/access code.
